Generally, the question asked in nclex-rn exam are multiple choice question in which you have to choose one answer among four choices. However, there are other alternate question asked in the exam such as fill in the blank, drop and drag question, choice more than one answer question.
The syllabus includes all the subject which you have studied in your nursing college. However, question pattern changes every five years. The topic asked in NCLEX-RN exam includes Safe and Effective Care Environment, Management of Care, Safety, Infection Control, Health Promotion and Maintenance Psychosocial Integrity, Physiological Integrity, Basic Care and Comfort, Pharmacological and Parenteral Therapies, Reduction of Risk Potential, Physiological Adaptation.

The question can be ranged from 75-250 question, it very from person to person that how many questions you will get in exams. If you are giving right answer than the difficulty level increase and then you will get more difficult question and your test will stop on an average of 85 question. However, if you are giving wrong answer than your question range can be extant to 250 questions. Additionally, the first 15 question for all students are given for pretest they do not affect your score whether you are giving right or wrong answer.
You can take nclex-rn examination for unlimited time. However, you have to take 60-day space between writing your next nclex-rn examination.
